This is a fully remote position, open to applicants in United States.
• Perform comprehensive assessments, identify challenges, create personalized care plans, and modify interventions to facilitate recovery and self-management throughout the care continuum.
• Coordinate behavioral health services, manage care transitions, implement crisis prevention strategies, and arrange aftercare with members, providers, and health plan medical management teams.
• Establish trusting, collaborative connections with members and providers through motivational interviewing, education, and continuous communication.
• Act as the primary point of contact for members, promoting information sharing, discharge planning, and linking to behavioral health, community, and support resources.
• Guarantee precise and timely documentation and service authorization in line with medical necessity criteria, regulatory standards, accreditation requirements, and internal policies.
• Engage in audits, quality improvement projects, case presentations, and continuous professional development.
• Foster high-quality care delivery, optimal resource utilization, member engagement, continuity of care, and recovery.
• Must hold a current, unrestricted state license to practice independently as a Clinical Social Worker, Marriage and Family Therapist, Professional Counselor, Clinical Psychologist, or Registered Nurse in the relevant state(s).
• Possess 3–5 years of direct clinical experience in facility-based or outpatient behavioral health or substance use treatment environments.
• Demonstrate strong clinical judgment and case management capabilities.
• Ability to assist members through intricate care transitions.
• Excellent verbal and written communication abilities, characterized by empathy and clarity.
• Proficient in Microsoft Outlook and Word.
• Capable of documenting in real-time while interacting with members.
• Skilled in interpreting and applying medical policies and clinical criteria.
• Must pass background checks upon hiring and throughout employment, including criminal, identity, education, employment, sanctions, watchlist, sex offender registry, and drug screening checks.
• Self-driven and responsible for achieving objectives.
• Ability to work independently to fulfill caseload demands and team goals.
• Professional development through specialization, certification, and/or advanced degrees is encouraged.
• High-speed cable or fiber-optic internet must be installed prior to starting, with a minimum download speed of 20 Mbps, upload speed of 5 Mbps, and maximum latency of 100 milliseconds.
• Must frequently operate computer and phone systems.
• Capacity to remain stationary and seated for prolonged periods, communicate, review information, perform repetitive motions, interpret data, problem-solve, make decisions, organize, plan, and maintain a positive professional demeanor.
• A quiet home workspace with minimal background noise is required.
• A fixed, designated workspace within the home is necessary.
• A hardwired internet connection is mandatory, along with the ability to utilize company-issued wired headsets and desktop equipment.
