This is a fully remote position, open to applicants in North Carolina.
• Receive and record all referrals from various sources into the Care Management documentation system.
• Confirm eligibility and demographic details.
• May conduct Health Risk Screenings as necessary.
• Assist in sending educational materials, consent forms, or other documents to members as required.
• Support the Care Management team with referrals.
• Provide information regarding access to and coordination of resources.
• Help members with care coordination and navigation of the healthcare system.
• Deliver culturally appropriate health education and information.
• Offer general education and social support.
• Act as an advocate for members.
• Help schedule medical appointments and arrange transportation as needed.
• Assist in addressing Social Determinants of Health as necessary.
• Access various Electronic Health Records (EHRs) to gather and upload information into the care management platform.
• Promptly notify the supervisor of any issues while performing assigned duties.
• Comply with CCNC (Community Care of North Carolina) Privacy and Security policies to ensure patient and company data is appropriately protected.
• Follow department guidelines, company policies, and HIPAA regulations.
• Perform additional duties to help maintain organized and functional operations.
• Attend departmental and corporate meetings, local and regional training, or other events as required.
• Understand and support CCNC goals, objectives, and standards.
• High school diploma or GED required, or Licensed Practical Nurse qualification.
• Minimum of 2-4 years of experience in a healthcare setting required.
• A 2 or 4-year degree in a health-related field is preferred.
• Bilingual candidates are preferred.
• Knowledge of and experience with patient or clinical data systems.
• Required computer skills, including various office software and internet proficiency; experience with MS Office software is preferred.
• Familiarity with state and federal benefits systems.
• Excellent oral and written communication skills.
• Proficient in motivational interviewing techniques.
• Strong organizational and time management skills.
• Sensitivity to cultural diversity, language barriers, health literacy, and educational levels.
• Understanding of medical terminology.
• Responds positively to change and demonstrates a willingness to learn new methods for achieving work objectives.
• Capable of adjusting strategies or approaches based on situational demands.
