Care Management Coordinator

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee and manage care services for patients

• Ensure delivery of high-quality, timely, and coordinated care

• Record care documentation in accordance with International Classification of Diseases (ICD) coding standards

• Collaborate closely with healthcare providers to formulate care plans

• Manage referrals and monitor patient progress

• Examine medical records

• Detect coding inconsistencies

• Offer education and guidance to both internal and external staff regarding ICD coding guidelines

• Address member needs and connect them with suitable service providers

• Execute procedures for confirming member identity, membership status, and eligibility for services

• Keep membership databases and records up-to-date and accurate

• Propose new service offerings or modifications to current programs based on member feedback or identified coverage gaps

• Engage in member retention or renewal initiatives

• Contribute to the realization of SCAN's Vision and Goals

⛳️ Requirements

• High School Diploma or equivalent experience

• 0-3+ years of relevant experience

• Performs tasks with minimal supervision

• Addresses complex issues and problems, escalating only the most challenging cases to senior staff

• In-depth knowledge of the subject matter

• Provides leadership, coaching, and/or mentoring to a subordinate group

• May serve as a lead or first-level supervisor

• Proficient in PC applications including Email, Word, Excel, and PowerPoint

• Basic technical skills relevant to the functional area

• Fundamental problem-solving skills

• Strong communication and interpersonal skills

• Proficient in both oral and written communication

• Excellent interpersonal abilities

• Customer/client focused

• Strong customer service skills

• Capable of problem/situation analysis

• Familiarity with ICD coding principles

• Strong attention to detail and a solid work ethic

• Background check required
