This is a fully remote position, open to applicants in Canada.
• Conduct comprehensive intakes and nursing evaluations to assess client needs, health history, social support systems, caregiver requirements, objectives, and suitable program/resource pathways.
• Create, document, and revise care coordination plans alongside clients and caregivers, outlining priorities, achievable goals, follow-up actions, and clear next steps.
• Assist clients, caregivers, and families through compassionate education, coaching, and motivational interviewing, ensuring they comprehend provider instructions, symptoms, care plans, treatments, records, and self-management needs.
• Guide clients through healthcare, community, social, benefits, workplace, wellness, travel, and virtual resources; facilitate appointment preparations, referrals, and follow-ups; identify gaps; and advocate respectfully for access, continuity, and client preferences.
• Coordinate and disseminate pertinent information with authorized providers, care team members, program partners, and internal colleagues promptly and in compliance with privacy regulations to support transitions, minimize duplication, and enhance continuity of care.
• Offer non-emergency guidance, regular check-ins, and monitoring; identify changes, symptom issues, mental health concerns, or caregiver challenges; and escalate for clinical review, provider follow-up, or emergency care as necessary.
• Utilize Serefin digital platforms, documentation tools, and secure communication channels to consolidate health information, manage caseload priorities, uphold confidentiality, and deliver high-quality care.
• Work collaboratively with colleagues and leadership to enhance care coordination resources, referral/resource libraries, workflows, digital tools, platforms, and communication pathways; engage in meetings, training, and quality improvement initiatives.
• Operate independently with sound judgment while remaining responsive, team-oriented, and in alignment with clinical standards, policies, and operational requirements.
• Assist in achieving the organization's operational objectives and contribute to other business initiatives as needed.
• Registered Nurse (RN), Registered Practical Nurse (RPN), or Licensed Practical Nurse (LPN) currently registered and in good standing with their respective regulatory body.
• A minimum of 3 years of nursing experience in acute care (e.g., Emergency, Medicine, ICU), community care, primary care, complex care, or a similar environment.
• Experience in nursing for the elderly population and individuals with complex needs is preferred.
• Strong skills in assessment, triage, documentation, teaching, and coaching.
• Capability to support clients, caregivers, and families through complex, sensitive, or emotional discussions with empathy, professionalism, cultural humility, and clear boundaries.
• Proven problem-solving, decision-making, and critical thinking abilities while effectively managing multiple priorities.
• Ability to work effectively both collaboratively and independently.
• Excellent interpersonal and communication skills, both verbal and written.
• Proficiency in English is mandatory. Fluency in French is highly preferred; knowledge of additional languages is an advantage.
• Comfortable utilizing technology, including electronic documentation platforms, Microsoft 365 applications, telephone systems, and other digital tools.
• Must have a private, secure workspace with a door that can be closed to ensure client privacy and confidentiality.
• Current Police Reference Check is required.
