Care Coordinator – Non-Clinical

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Manage incoming phone calls and return calls from patients related to clinical inquiries for designated providers, perform protocol-based triage as necessary, and document calls in the EHRS as phone notes for provider review and guidance.

• Make calls and generate template-based letters as instructed by the provider to communicate lab results to patients promptly.

• Review the daily schedule to confirm that all pre-visit preparations are finalized ahead of the patient's appointment, ensuring that lab and imaging results, consultation reports, care transition details, or significant changes in health status are accessible in the patient’s medical record.

• Contact patients identified as at risk for "no-shows" before their appointment, as well as new patients establishing care for their onboarding visit, to remind them of both the patient’s and Imagines’ responsibilities.

• Facilitate the completion of forms for provider review and final signature (such as disability forms, home health orders, durable medical equipment and supply requests, and disease status letters).

• Support the care team in the regular preparation of reports to help manage and evaluate the health needs of the patient population.

⛳️ Requirements

• High school diploma or equivalent.

• 2-4 years of healthcare experience in outpatient clinic settings.

• Experience in specialty/multispecialty clinics is highly preferred.

• Fluency in Spanish is preferred.
