This is a fully remote position, open to applicants in United States.

📋 Description

• Assist in care management activities and collaborate with assigned member teams to ensure the delivery of provider and partner services while achieving continuity of care and member satisfaction.

• Conduct member outreach via phone calls or home visits.

• Record care/service plans and related activities.

• Engage with members to review care plans, discuss next steps, resources, concerns, recommendations for care, and ongoing education.

• Coordinate care activities involving healthcare providers, community partners, members, and caregivers.

• Address inquiries, requests, and concerns from members and providers related to care/service plans.

• Communicate effectively with care managers, practitioners, and others to facilitate member services and ensure continuity of care.

• Assist with service assessments/screenings and document the care needs of members.

• Maintain member records in compliance with state and regulatory requirements and share them with providers as necessary.

• Adhere to contractual, regulatory, and organizational standards and policies.

• Identify member needs and make appropriate referrals to Care Managers, community-based organizations, and Disease Managers.

• Provide information on available benefits and resources.

• Perform additional duties as assigned.


⛳️ Requirements

• High School diploma or GED is required.

• 1–2 years of relevant experience is required.

• For Arkansas Total Care: a bachelor's degree in a social science or health-related field, or a high school diploma with at least 1 year of experience coordinating care for clients with developmental or intellectual disabilities or behavioral health issues.

• Must reside in Arkansas or a bordering city.

• A valid driver's license is required.

• Child and adult maltreatment checks are mandatory prior to hiring and on a recurring basis.

• A drug screening is required at the time of hiring and on a recurring basis.

• Ability to support members throughout Craighead and Poinsett counties in Arkansas.

• Strong experience working with the developmental disability population is preferred.

• Field-based work experience supporting members with behavioral health and developmental disability needs.

• Willingness to travel 30%; additional travel may be necessary throughout Greene and Mississippi counties.

• Compliance with applicable policies, contractual obligations, and regulatory guidelines is essential.
