This is a fully remote position, open to applicants in United States.
• Assist in care management tasks and collaborate with designated member care teams
• Conduct outreach to members through phone calls and home visits
• Review care plans and service plans, discuss next steps, resources, questions, and concerns
• Provide ongoing education to members throughout their care and services
• Facilitate coordination of care among healthcare providers, community organizations, partners, members, and caregivers
• Address inquiries, requests, and concerns from members and providers
• Engage with care managers, practitioners, and others to ensure member services and continuity of care
• Assist with service assessments and screenings, documenting member care needs
• Maintain and document member records in accordance with state and regulatory standards
• Recognize needs and refer to Care Managers, community organizations, and Disease Managers
• Educate members about available benefits and resources
• Carry out additional duties as assigned
• High School diploma or GED
• 1–2 years of relevant experience
• For Arkansas Total Care: Bachelor's degree in a social science or health-related field, or a high school diploma with a minimum of one year of experience in coordinating care for clients with developmental or intellectual disabilities or behavioral health issues
• Significant experience working with the developmental disabilities population
• Must reside in Arkansas or a bordering city
• Ability to support members in Hempstead and Howard counties in Arkansas
• Valid driver's license required
• Child and adult maltreatment checks must be completed prior to hiring and periodically thereafter
• Drug screening is required at the time of hiring and periodically thereafter
• Willingness to travel 30% of the time and possibly throughout Little River and Sevier counties
