Care Coordinator II

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Assist in care management activities and collaborate with designated member teams to guarantee the provision of services and maintain continuity of care/member satisfaction.

• Perform member outreach through phone calls and home visits.

• Document care/service plans along with related activities.

• Engage with members to discuss care plans, next steps, available resources, inquiries, concerns, and ongoing education.

• Coordinate care activities with healthcare professionals, community providers, partners, members, and caregivers.

• Address member and provider inquiries, requests, and concerns.

• Communicate with care managers, practitioners, and others to facilitate member services.

• Assist in service assessments and screenings, documenting member care needs.

• Maintain member records in compliance with state and regulatory standards.

• Identify member needs and refer them to Care Managers, community-based organizations, and Disease Managers.

• Provide information about available benefits and resources.

• Carry out additional duties as assigned while adhering to policies and standards.

⛳️ Requirements

• A High School diploma or GED is mandatory.

• 1–2 years of relevant experience is required.

• Must be a resident of the state of Missouri.

• Capability to conduct telephonic outreach and home visits.

• Proficiency in documenting care needs and maintaining member records.

• Ability to identify needs and make referrals effectively.

• Understanding of care/service plans, benefits, and available resources.

• Ability to adhere to state, contractual, and regulatory requirements.
