This is a fully remote position, open to applicants in Illinois.

📋 Description

• Conduct home visits for Medicaid members across various settings including their residences, Assisted Living Facilities, and Long-Term Care Facilities.

• Evaluate members' care requirements and ensure that services are delivered in the least restrictive environment.

• Identify and address obstacles to effective care delivery.

• Develop and execute strategies to meet care needs.

• Coordinate services while monitoring and assessing case management plans in relation to members’ personal objectives.

• Regularly track patient care and progress towards desired outcomes through assessments, data analysis, discussions, and proactive care planning.

• Perform both telephonic and in-person assessments and evaluations.

• Address immediate and acute care gaps and accessibility issues.

• Assist enrollees in the process of filing grievances and appeals.

• Provide information regarding MCO requirements, services, and benefits.

• Offer guidance and referrals to community resources.

• Steer members and their families towards suitable care resources.

• Facilitate communication among payer sources, providers, interdisciplinary teams, and other stakeholders involved in members’ care.


⛳️ Requirements

• A Bachelor’s degree in social sciences, social work, human services, or a related discipline; or an unrestricted Illinois LPN with a minimum of one year of experience in conducting comprehensive assessments and delivering formal services for the elderly; or LPC; or LSW.

• Must be a resident of Illinois within one of the designated eligible zip codes.

• At least 1 year of experience in healthcare and/or case management.

• A minimum of 1 year of experience working with Medicare and Medicaid recipients, long-term care, home and community-based services, and/or managed care organizations.

• Intermediate to advanced proficiency in computer skills.

• Familiarity with Microsoft Word, Excel, and Outlook.

• Experience with electronic information applications/software, including electronic medical records.

• A valid state driver's license.

• Maintain personal vehicle liability insurance that meets state minimum requirements or Humana's specified limits, whichever is greater.

• Successfully pass the required tuberculosis screening if selected.

• A dedicated home workspace free from ongoing interruptions to safeguard member PHI/HIPAA information.

• High-speed home internet with a minimum of 25 Mbps download and 10 Mbps upload speeds.

• Availability to work Monday–Friday, 8am–5pm CST, with the flexibility for potential additional hours based on business needs.
