Care Coordinator

This is a fully remote position, open to applicants in Florida, +2 more states.

📋 Description

• Facilitate the completion of Comprehensive Health Evaluations (CHE) in nephrology offices, dialysis clinics, or through telehealth options.

• Collaborate with Nurse Practitioners and clinics to effectively prepare for in-person appointments.

• Make outbound calls to patients, caregivers, and clinics for scheduling, reminders, and rescheduling of CHEs.

• Arrange telehealth CHEs for patients who are unable to attend in person.

• Greet new patients and gather concerns from current patients, escalating to RN or Nurse Practitioner as needed.

• Keep precise documentation and relay patient updates to the care team.

• Organize outreach initiatives based on urgency and significance.

• Ensure coordinated care by facilitating access to eligible resources and programs while assisting with referrals and provider appointments.

• Conduct both outbound and inbound communications with patients, providers, and Village resources.

• Carry out non-clinical screenings and pathways under the guidance of the Manager, escalating to the Integrated Care Nurse as necessary.

• Support the clinical team in guiding patients through the continuum of care.

• Identify eligible resources and work with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to enroll members in relevant programs.

• Communicate directly with patients in a prompt and professional manner.

• Maintain accurate, complete, and confidential documentation.

• Provide exceptional patient care in alignment with DaVita’s mission and values.

⛳️ Requirements

• Must have prior experience as an IKC Care Coordinator or Engagement Specialist.

• High School Diploma or GED is required.

• Preferred 2+ years of professional experience in customer service, patient-facing, or telephonic outreach roles.

• Previous healthcare experience is preferred.

• Intermediate skills in Microsoft Word, Excel, PowerPoint, and Outlook are necessary.

• Exceptional verbal and written communication abilities, along with active listening and motivational engagement techniques.

• Highly organized with outstanding time management skills and follow-through.

• Capable of working both independently and collaboratively with care team members.

• A home workspace that offers a quiet, private environment with reliable high-speed internet is required.
