Care Coach

This is a fully remote position, open to applicants in Arizona, +25 more states.

📋 Description

• Foster program engagement by articulating the benefits of Maven’s care coaching model, enhancing opt-in rates, and facilitating sustainable behavior change.

• Collaborate with members to establish SMART goals, monitor adherence, modify care plans, and make necessary adjustments within Maven’s clinical program framework.

• Manage a caseload through continuous messaging, video consultations, and proactive outreach.

• Adhere to clinical program guidelines and protocols while employing motivational interviewing and behavior modification techniques.

• Evaluate member knowledge and obstacles, offering tailored health education.

• Partner with providers to translate clinical recommendations into practical next steps.

• Remotely track member health data and platform activity to identify risks and take appropriate measures.

• Coordinate care across Maven’s digital network, in-person provider networks, and other external partners.

• Engage in and spearhead initiatives to enhance the quality of care and clinical outcomes.

• Provide coverage to ensure members receive timely care and responses.

⛳️ Requirements

• 1–2+ years of experience in health coaching or clinical roles, such as nursing or behavioral health, with a focus on patient education, motivational interviewing, and clinical outcomes.

• Experience with maternity populations and/or individuals navigating family planning or fertility care.

• Training and experience in coaching individuals towards health outcomes, including medication management, nutrition and exercise goals, or blood pressure management.

• Adaptability to changes, efficient work to meet member demands, and a growth mindset.

• Compassionate written and verbal communication with patients, demonstrating a commitment to a positive member experience.

• Proven track record of achieving patient-outcome KPIs, including opt-in rates, follow-up adherence, and member satisfaction.

• Capability to learn and adjust to digital health tools and platforms in a rapidly evolving technological landscape.

• Openness to using AI tools in daily tasks.

• Strong telehealth skills, including phone outreach and video consultations.

• Reliable access to high-speed internet.

• Health coaching certification (NBC-HWC or ICF) is preferred, but not mandatory.

• Experience in a start-up or fast-paced environment is preferred, but not mandatory.

• Background in coaching via video, text/chat, and phone is preferred, but not mandatory.

• Familiarity with Google Suite, CRM platforms, EHRs, and Zendesk is preferred, but not mandatory.

• Active work authorization in the US.
