This is a fully remote position, open to applicants in Pennsylvania.
• Implement personalized care plans for patients facing serious and high-risk medical conditions.
• Serve as the intermediary between patients and billing professionals.
• Perform individualized assessments to gain insight into the patient's social and cultural environment.
• Assist patients and their caregivers in navigating the healthcare system in Pennsylvania.
• Facilitate the use of patient communication templates and automation tools.
• Connect patients with supportive services and community resources.
• Employ motivational interviewing techniques and capacity-building strategies to aid patients in managing the pressures of chronic illness.
• Maintain comprehensive records of all interactions, detailing the time invested and specific activities undertaken.
• Community Health Worker (CHW) Certification is preferred; candidates with a background in CHW or those who have completed CHW training are highly encouraged to apply.
• Compliance: Must be willing to undergo and uphold all CMS-required training and state-specific mandates, including competencies in interpersonal skills, service coordination, and ethical professional conduct.
• Experience: A minimum of 3 years in community outreach, facilitation, or healthcare is required. Experience with high-risk chronic conditions or complex care management is a significant advantage.
• Technical Savvy: Proficient computer skills and the ability to use web-based and app-based systems effortlessly.
• Cultural Competency: A thorough understanding of the socio-economic and public health issues affecting marginalized populations in Pennsylvania is essential.
• Communication: Exceptional written and verbal communication abilities, coupled with an engaging personality and the capacity to inspire and influence diverse groups.
• Logistics: A high school diploma or GED is required (a college degree is preferred). A valid driver’s license, dependable transportation, and a flexible schedule for occasional work outside standard hours are necessary.
