This is a fully remote position, open to applicants in Alabama, +7 more states.
• Oversee a group of patients receiving remote care, making timely clinical decisions grounded in evidence-based guidelines for conditions such as heart failure, hypertension, and type 2 diabetes.
• Provide high-quality, evidence-based care by adhering to established clinical protocols and workflows while utilizing sound clinical judgment tailored to individual patient needs.
• Initiate and adjust guideline-directed medical therapy (GDMT) in close collaboration with physicians, RNs, and care teams to enhance patient outcomes.
• Respond swiftly to clinical escalations arising from remote patient monitoring, which includes abnormal vital signs, symptoms, and lab results.
• Carry out both synchronous and asynchronous patient care, which involves reviewing patient-generated health data, addressing remote patient monitoring alerts, utilizing secure messaging for communication, and conducting scheduled virtual visits as needed.
• Deliver safe, efficient, and high-quality care while consistently meeting established productivity and documentation standards.
• Adapt swiftly to changing workflows, software updates, and operational processes as we continuously enhance our care model.
• Confidently use multiple technology platforms and provide feedback that contributes to the improvement of our clinical operations and technology.
• Collaborate across clinical and operational teams to identify opportunities for enhancing patient care, efficiency, and scalability.
• Play a role in shaping the future of a rapidly expanding digital health organization where every team member helps in building improved processes rather than merely following existing ones.
• Active Compact Multi-State RN license.
• Active NP licensure in at least two of the following states: TX, MI, NC, CA, PA, NJ, CT, AL. Exceptional candidates with fewer licenses may be considered based on overall qualifications and capability to obtain additional priority state licenses.
• 5+ years of experience treating patients with chronic diseases (CHF, Hypertension, T2D) in either outpatient or inpatient settings at a high-performing medical center.
• 3+ years of cardiology experience with a focus on heart failure management and guideline-directed medical therapy (GDMT).
• Active ANCC or AANP certification.
• Master’s Degree as a Nurse Practitioner with an openness to expand state licensure as Cadence introduces new markets and partners.
• Home office setup with consistently stable Wi-Fi featuring strong upload and download speeds. A Wi-Fi speed test is required during the interview process to confirm compliance with these standards.
• Proven success in fast-paced environments where priorities, workflows, and technology frequently evolve.
• Comfort in receiving regular feedback and continuously adapting processes.
• Highly organized with excellent time management skills, capable of balancing patient care with productivity expectations.
• Self-motivated and accountable while working remotely with minimal supervision.
• Curious, resilient, and excited about developing new methods of delivering care.
• Preferred experience in a CHF bridge clinic or T2D clinic, in a startup environment, and with remote patient monitoring technology.
• Comfortable utilizing AI-assisted clinical tools (such as Gemini, Open Evidence, or similar platforms) while maintaining clinical judgment as the primary guide for patient decisions.
