Bilingual Spanish Telephonic RN Nurse Case Manager – Special Needs Plan

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver telephonic case management to members with complex medical conditions and chronic illnesses.

• Perform thorough health assessments and develop personalized care plans.

• Facilitate care coordination with both internal and external partners, including physicians and specialists.

• Provide education to members and caregivers regarding disease management and preventive healthcare.

• Track member progress and advocate for prompt and appropriate interventions.

• Identify and assist in resolving service or access challenges affecting the quality of care.

⛳️ Requirements

• Possess an active, unrestricted RN license in California (Non-Compact).

• Have a minimum of 2 years of clinical nursing experience.

• Hold at least 1 year of experience in case management.

• Must be bilingual (English-Spanish).

• Proficient in Microsoft Office applications (Word, Excel, Outlook).
