Behavioral Health Case Manager

📋 Description

• Evaluate the needs of members across various domains, including physical health, mental health, substance use disorder (SUD), oral health, palliative care, memory care, trauma-informed care, social supports, housing, and connections to community-based services.

• Supervise the formulation of client care plans and the establishment of goals.

• Provide services in locations convenient for members, whether at home, in an office, or through telehealth and field-based options.

• Link clients to essential social services and supports as required.

• Advocate for clients with healthcare professionals, such as primary care providers.

• Implement evidence-based practices, including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles.

• Conduct outreach and engagement initiatives to promote access to the ECM program and document activities in the Client Relationship Management (CRM) system.

• Assess clients' progress and revise SMART goals accordingly.

• Facilitate mental health promotion activities.

• Organize transportation services, such as ACCESS.

• Ensure timely completion of all documentation, including outcome measures, as outlined in individual care plans.

• Keep patient health records current in the Electronic Medical Record (EMR) system and other relevant business systems.

• Prepare monthly reports to ensure compliance with program requirements.

• Participate in assigned training sessions.


⛳️ Requirements

• Must possess an active, unrestricted ACSW, LCSW, LMFT, or LPCC license in California.

• A minimum of one year of experience as a care manager, care navigator, or community health worker serving vulnerable populations.

• Familiarity with government and community resources related to social determinants of health is essential.

• Strong oral and written communication abilities are required.

• Positive interpersonal skills are necessary.

• Basic computer skills and proficiency in Google Workspace, MS Office, and internet usage are a must.

• Bilingual proficiency in English and Spanish is highly preferred.
