Ambulatory Clinical Pharmacist

This is a fully remote position, open to applicants in Colorado.

📋 Description

• Deliver integrated support to both patients and physicians through a hands-on approach aimed at enhancing the quality of patient care.

• Promote advancements in quality pharmacotherapy by ensuring the delivery of safe, effective, and cost-efficient drug therapy.

• Work in close collaboration with the hybrid ACP team to fully utilize the comprehensive suite of therapy management (TM) clinical services within the designated clinics.

• Conduct in-person and/or virtual assessments focused on disease state management and drug therapy education, tailored according to medication risk level and approved clinic protocols.

• Strive to identify, resolve, and prevent issues related to medication.

• Provide additional services to address barriers to medication compliance and other adherence-related challenges.

• Monitor and assess medication regimens to ensure adherence to clinical criteria, including therapeutic objectives, side effects, and drug interactions.

• Identify opportunities for optimizing drug therapy and offer recommendations for medication cost-saving initiatives when appropriate.

• Engage in quality improvement efforts by actively participating in clinical outcome studies, REMs compliance/reporting, and ACP-related process enhancement projects.

• Enhance resource utilization and workflows by supporting roles that manage complex prior authorizations/appeals, providing clinical justification for medication approvals, including supporting literature and rationale.

• Collaborate with healthcare providers, including assigned clinic physicians, specialists, and onsite clinic pharmacists, to ensure efficient medication management and care coordination.

⛳️ Requirements

• Candidates must hold a Doctor of Pharmacy degree (PharmD) from an accredited pharmacy school.

• Must be a licensed pharmacist in Colorado.

• Additionally, candidates must meet at least one of the following criteria:

• Have certification in a relevant area of practice.

• Completed a postgraduate residency at an accredited institution, with a minimum of 50% of the experience involving direct patient care services alongside interdisciplinary teams.

• Have provided clinical services to patients for no less than one year under a collaborative practice agreement or protocol with a physician, advanced practice pharmacist, or as part of collaborative drug therapy management, hospital pharmacy practice, or health systems.

• Candidates must fulfill the necessary continuing education requirements to maintain their Colorado state Pharmacist license if selected for the role.

• Ability to analyze information, demonstrate sound judgment, possess strong verbal communication skills, and have extensive knowledge of pharmacological advancements and FDA health regulations. Must be able to manage processes, showcase robust technical and clinical skills, maintain a constant focus on legal compliance, while demonstrating high productivity with an emphasis on quality and detail. Strong clinical knowledge and validated expertise with an emphasis on disease state management and treatment guidelines.
