This is a fully remote position, open to applicants in Ohio.
• Deliver Comprehensive Medication Management (CMM) in partnership with hub-based and virtual clinical care teams, along with providing additional pharmacist support services as required.
• Assist in formulating a medication management action plan aimed at achieving member self-management objectives and clinical outcomes, with defined parameters and timelines for monitoring and follow-up.
• Collaborate and communicate effectively with Cityblock and external providers within the member care team to ensure that all clinical interventions and interactions align with medication-related objectives, while conducting ongoing evaluations and adjustments to the care plan as needed to facilitate goal achievement.
• Offer direct support to clinical team members regarding all general drug information, dosing, interaction/adverse effect avoidance inquiries, or member-specific needs.
• Engage in staff education by presenting medication management in-services, creating educational materials for both clinical and non-clinical team members, and training/coaching/facilitating all roles on the consistent upkeep of a member’s optimal medication list care management platform, Commons, as well as the post-discharge medication reconciliation process for providers.
• Aid in developing medication management and disease state-specific clinical pathways/protocols, with the ultimate aim of transitioning to collaborative practice agreements (CPA) with Cityblock physicians, allowing pharmacists to conduct member assessments, order laboratory tests, and initiate or adjust medications as per protocol to enhance the efficiency and effectiveness of team-based care.
• Completion of a PGY1 pharmacy practice residency, along with a PGY-2 Ambulatory Care Pharmacy Residency, or a minimum of 2 years of experience as a clinical pharmacist in an ambulatory care environment.
