This is a fully remote position, open to applicants in Connecticut, +12 more states.
• Serve as the representative for the Alumni Services Department across all 11 agencies.
• Participate in agency territorial and departmental meetings within the designated territory.
• Keep the territory informed regarding Alumni announcements.
• Collaborate and communicate with agency leadership and care teams to facilitate seamless client transitions into the Alumni Community post-discharge.
• Evaluate client tracking reports to assess alumni eligibility.
• Ensure that the discharged client information in the EMR is accurately reflected in the post-discharge tracking and engagement Salesforce dashboard.
• Organize client introductions to the Alumni Community upon request.
• Clearly articulate the details of the Alumni Community, ensuring clients are aware of eligibility criteria and guidelines.
• Conduct post-discharge follow-up calls and send informational emails.
• Collaborate with admissions when former clients or current alumni seek assistance in finding treatment or rejoining the program.
• Work alongside the communications department to gather testimonials from former clients.
• Coordinate alumni community events and meetings.
• Partner with the CaredFor team to address app-related inquiries or issues.
• Collaborate with the Business Development team when former clients seek support in locating an external provider.
• Minimum of one year experience working with patients in the Substance Use Disorder (SUD) sector.
• An Associate’s Degree or higher is preferred.
• Candidates must reside in CT, FL, GA, IN, KY, ME, MA, NH, OH, RI, VA, NJ, NY, or PA, TX.
• Preferred: 2 years of lived experience in sustained recovery from a Substance Use Disorder and/or personal experience with addiction involving family members or loved ones.
• Proficient in Microsoft applications, including Outlook, Teams, Excel, and Word.
• Strong familiarity with Salesforce CRM.
• Solid understanding of EMR software.
• Willingness to travel is required.
