ALF Case Manager

This is a fully remote position, open to applicants in California.

📋 Description

• Verify Medi-Cal eligibility for each client on a monthly basis.

• Arrange monthly appointments with clients.

• Communicate with clients to determine any needs, such as DMEs, appointments, etc.

• Inquire about medication adherence and other general assessment questions.

• Coordinate with staff to identify any SIRs. If present, ensure our office receives them. If any staff members are absent without an SIR, inform them that obtaining one is urgent. Follow up with facility staff to ensure our office receives a copy of the SIR.

• If there are any Medi-Cal concerns, inform the client and facility staff, providing guidance on how to resolve these issues if they are unaware.

• Discuss medications with staff and review the log.

• Update client information in the company tracking system regarding visit dates and when notes are uploaded in Devero. All notes must be uploaded by the end of the next business day following the client visit. Document in your notes that you inquired about medication compliance with both the client and staff.

• According to the workflow process, when a client is out of a facility, the CCA has specific tracking responsibilities. As an ALF Case Manager, you are responsible for monitoring when clients leave during your facility visits.

• Inform the ALF Coordinator when a client has departed from and returned to the facility.

• Upon completion of a Re-Assessment, the RN will notify you via email. You are required to make a copy of the ISP for the client to sign and provide a copy to the facility. Afterward, upload the document to Devero and email your CCA to confirm completion. Ensure the client signs within 7 business days.

• Possess knowledge of confidentiality, HIPAA, and healthcare laws and regulations.

• Maintain accurate timekeeping records.

• Adhere to policies and procedures outlined in the Employee Handbook.

• Keep all credentials up to date.

• Report any instances of fraud and abuse.

• Understand mandated reporting requirements.

• Conduct timely recording and documentation of client interactions.

• Participate in Department meetings (in person, via phone, or online).

• Attend all state-mandated in-service training sessions.

• Driving may be necessary to reach geographical areas serviced by the company, with significant driving required throughout Los Angeles, Fresno, San Diego, Riverside, and San Bernardino County.

• Carry out additional duties as assigned.

⛳️ Requirements

• Bachelor’s Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology, along with one year of relevant work experience.

• Hold current CPR certification.

• Preferably knowledgeable about The State of California Assisted Living Waiver Care Coordination Agency.

• Experience in education, training, quality assurance, and management is preferred.

• Demonstrate excellent judgment and understanding of current clinical practices, with proven clinical, analytical, and documentation skills.

• Possess strong organizational skills.

• Be self-directed and capable of working with minimal supervision.

• Proficient in using computers and electronic information systems.

• Ability to establish and maintain effective communication and relationships with office, field, and administrative personnel.

• Familiarity with Title 22 is preferred.

• Must be a licensed driver with a properly insured automobile in good working condition, in compliance with state and/or organizational requirements.
