This is a fully remote position, open to applicants in Arizona.

📋 Description

• Deliver exceptional care services remotely through telehealth for patients and families residing in home settings.

• Navigate and manage complex chronic comorbidities effectively.

• Develop and implement treatment plans tailored for patients and their family caregivers.

• Collaborate with the transdisciplinary team to create personalized care plans.

• Monitor patient progress and modify care plans as needed.

• Clinically supervise and work alongside Care Navigators in managing a shared patient panel, offering real-time clinical guidance, consultation, and support.

• Review, approve, and maintain patient charts, documentation, and individualized care plans to ensure high-quality, compliant care.

• Complete precise clinical documentation and billing for Chronic Care Management (CCM) and other fee-for-service programs, ensuring adherence to payer requirements and organizational standards.

• Collaborate with fellow healthcare professionals to guarantee coordinated holistic care.

• Stay informed about current trends and advancements in geriatric care.

• Conduct screenings and assessments for social determinants of health (SDOH) and behavioral needs.

• Provide clinical and medical education to clients and staff.

• Participate in virtual care conferencing.

• Update and maintain medical records.

• Engage in quality improvement initiatives and aid in the development of best practices.

• Contribute to the strategic direction of Nolia Care services, a leader in the field.


⛳️ Requirements

• A minimum of two years of outpatient experience in an independent private practice or a larger medical center.

• Preferred experience as a Family Nurse Practitioner (FNP) and Adult Gerontology Primary Care Nurse Practitioner (AGPCNP).

• Required experience utilizing chronic, longitudinal care models in digital health.

• Must possess a current, unrestricted Advanced Registered Nurse Practitioner (ARNP) license in Arizona.

• Strongly preferred additional active NP and RN licenses in Texas, Idaho, Minnesota, Michigan, and Pennsylvania.

• Board certification as a Nurse Practitioner in geriatric care is preferred, and a DEA is required.

• National board certification as a Nurse Practitioner from an accredited agency.

• A Master’s level ARNP is required, with additional training or experience in Geriatric care, Dementia, and Palliative specialties.

• A Doctorate of Nursing is preferred.
