Advanced Practice Provider

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide comprehensive virtual medical weight loss services, including patient assessments, diagnoses, treatment planning, chronic condition management, and follow-up care.

• Work collaboratively with a multidisciplinary team of dietitians, care navigators, case managers, behavioral health professionals, and external specialists to ensure integrated whole-person care.

• Order and analyze diagnostic tests, prescribe suitable medications and evidence-based treatment plans, and modify care strategies as clinically required.

• Involve patients in shared decision-making by clearly outlining treatment options, associated risks, and benefits while encouraging adherence to prescribed treatments and follow-up care.

• Provide culturally sensitive, patient-focused counseling that promotes lasting lifestyle changes and ensures a safe, respectful environment for patients to discuss their challenges and achievements.

• Communicate effectively and compassionately through synchronous telemedicine appointments and asynchronous messaging, addressing patient inquiries promptly and thoroughly.

• Participate in regular clinical team meetings and care conferences, contributing to case discussions, continuous education, and practice engagement.

• Coordinate care and share relevant patient information with local healthcare providers/organizations while adhering to HIPAA and state telehealth regulations.

• Keep thorough and timely documentation in the EHR, ensuring patient records are clear, compliant, and up-to-date.

• Promote operational excellence by performing essential administrative tasks and adhering to collaborative practice agreements and state-specific telehealth regulations.

⛳️ Requirements

• Certification or training in obesity medicine.

• Prior experience in telemedicine.

• Multi-state licensure (either compact or in multiple states) - preferred 10+ states.

• Understanding of value-based or outcomes-oriented care models.

• Team-oriented and aligned with the organization's mission.

• Exceptional presentation skills suitable for both clinical and non-clinical audiences.

• Self-motivated with the ability to work independently and achieve results with disciplined follow-through.
