Advanced Practice Clinician, NP/PA, Care Anywhere

This is a fully remote position, open to applicants in California.

📋 Description

• Perform in-home evaluations for Alignment members, including medication assessments and health screenings.

• Educate patients by aligning their desired care with the most suitable options available.

• Identify diagnoses for evaluation within care management and ongoing medical oversight.

• Collaborate and communicate with the Regional Medical Officer and other team members to formulate the most suitable care plans tailored to our members' needs.

• Facilitate care coordination with various stakeholders, which may include primary care physicians, specialists, and ancillary service providers.

• Lead a comprehensive clinical team that may consist of nurses, health coaches, social workers, and care coordinators.

⛳️ Requirements

• Required: Master’s degree from an accredited Nurse Practitioner or Physician Assistant program.

• Preferred: At least one (1) year of prior clinical or home care experience; previous experience with Electronic Medical Records (EMR) is also preferred. Experience in geriatric patient care is advantageous.

• Required: Strong ability to communicate in a positive, professional, and effective manner. Possess problem-solving skills along with effective organizational and time management abilities, capable of thriving in a fast-paced setting.

• Licensure: • Required: Active Nurse Practitioner or Physician Assistant license, along with an active RN license and furnishing number (mandatory upon hire). For Nurse Practitioners: Active Nurse Practitioner Board Certification/Physician Assistant Certification, NPI Number, DEA registration, valid BLS certification, a valid driver’s license, and current automobile insurance.
