This is a fully remote position, open to applicants in California.
• Perform in-home assessments for Alignment members, including medication reviews and health screenings.
• Educate patients by aligning their care preferences with the most effective healthcare solutions.
• Determine diagnoses for evaluation in care management and ongoing medical supervision.
• Collaborate and communicate with the Regional Medical Officer and other team members to create the most suitable care plans tailored to the needs of our members/patients.
• Facilitate care coordination among various stakeholders, including but not limited to primary care physicians (PCPs), specialists, and ancillary providers.
• Oversee a broader clinical team which may consist of nurses, health coaches, social workers, and care coordinators.
• Required: Master’s degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program.
• Required: Valid Nurse Practitioner or Physician Assistant license.
• Active RN License and Furnishing number (mandatory upon hire).
• For NPs: Active Nurse Practitioner Board Certification or Physician Assistant Certification.
• NPI Number, DEA registration, and current BLS certification.
• Valid Driver's License and up-to-date automobile insurance.
• Preferred: One (1) year of previous clinical or home care experience.
• Prior experience with EMR systems is preferred.
• Experience in caring for older adult (geriatric) patients is preferred.
