Advance Practice Provider, Nurse Practitioner – Bilingual

This is a fully remote position, open to applicants in New York.

📋 Description

• Deliver primary and urgent healthcare to patients via telephone and tele-video.

• Create and execute clinical care plans for adult patients experiencing chronic and complex conditions.

• Collect patient histories, conduct physical examinations, and obtain necessary diagnostics.

• Formulate and implement treatment plans that align with patient objectives and current health status.

• Recognize and address gaps in care.

• Attend to the physical and psychosocial needs of patients and their families with the support of an interdisciplinary team.

• Inform patients and families about medications, potential side effects, disease progression, dietary needs, adherence, and crisis prevention strategies.

• Coordinate care and resources with clinical team members, physicians, and medical providers.

• Maintain accurate patient medical records and clinical documentation.

• Engage in necessary continuing education.

• Prescribe medications as permitted by state regulations.

• Collaborate with company physicians, other healthcare providers, and family members to formulate complex care plans.

• Offer both clinical and non-clinical support to patients receiving home and community-based services.

⛳️ Requirements

• Bilingual or multilingual skills are mandatory.

• A Master of Science in Nursing is required.

• An active national Nurse Practitioner certification is required.

• A valid, current, and unrestricted Family or Adult Nurse Practitioner license in New York is required.

• A valid, current, and active RN license is required.

• A minimum of 2 years of experience in managing complex care cases is necessary.

• Familiarity with Electronic Medical Records (EMR) is required.

• Bilingual in Spanish is preferred.

• Medicaid registration is preferred.

• DEA registration or eligibility is preferred.

• Experience in managing complex care cases for patients with developmental disabilities and chronic illnesses is strongly preferred.

• Candidates must live within commuting distance of an office unless an accommodation is provided.

• Availability to work Monday through Friday, from 8:00 a.m. to 5:00 p.m. local time, with on-call coverage of 2 to 3 nights every 4 to 6 weeks and on holidays is required.

• New candidates in specific patient/member-facing roles must be vaccinated against COVID-19 and influenza unless an acceptable reason is provided.
