This is a fully remote position, open to applicants in Massachusetts.
• Deliver primary and urgent healthcare through telephone and tele-video consultations for patients receiving home and community-based services.
• Create and execute clinical care plans for adult patients with chronic and complex conditions.
• Collect patient history, conduct physical examinations, and perform diagnostics as necessary.
• Formulate and implement treatment plans that are in line with patient objectives and current health conditions.
• Identify and address gaps in care.
• Attend to the physical and psychosocial needs of patients and their families with the support of an interdisciplinary team.
• Educate patients and families on medication use, potential side effects, disease progression, nutrition, adherence, and crisis prevention strategies.
• Coordinate care and resources with company physicians, other medical professionals, patients, and families.
• Maintain patient medical records and ensure accurate medical documentation.
• Engage in mandatory continuing education.
• Prescribe medications as permitted by state regulations.
• Collaborate in developing complex care plans that align with patient health status, goals, and values.
• Master of Science in Nursing (MS in Nursing).
• Active national Nurse Practitioner certification.
• A valid, current, active, unrestricted Family or Adult Nurse Practitioner license in Massachusetts.
• A valid, current, active RN license.
• Over 2 years of experience in managing complex care cases.
• Proficient in working with Electronic Medical Records (EMR).
• Availability from Monday to Friday, 8:00 am to 5:00 pm local time.
• On-call availability for 2–3 nights every 4–6 weeks and holiday coverage.
• Registration with Medicaid is preferred.
• DEA registration or eligibility is preferred.
• Strong preference for experience in managing complex care cases for individuals with developmental disabilities and chronic illnesses.
• Ability to provide care in compliance with state regulations and company health standards and policies.
• COVID-19 and Influenza vaccinations are required for specific patient/member-facing roles unless an acceptable exemption is provided.
