Virtual Clinician – Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Colorado.

📋 Description

• Conduct approximately 30-minute online Video Visits with health plan members.

• Review patients' medications and medical histories.

• Educate members regarding their health and address any questions they may have.

• Assist in closing care gaps for Medicare Advantage, Medicaid, and specific commercial plan members.

• Ensure 40 hours of availability each week and manage over 320 capacity units monthly.

• Consistently achieve a minimum of 220 visits per month.

• Maintain training, affiliation, and compliance with Video Visit standards.

• Acquire training and credentialing for health plan clients and relevant lines of business in primary coverage states.

• Participate in regular meetings with management, team members, and clinical staff.

• Meet the operational, clinical, and member experience KPIs and metrics set by Signify Health.

• Provide telehealth services while ensuring the privacy of members and adhering to remote-visit standards.

⛳️ Requirements

• Nurse Practitioner or Physician Assistant qualifications.

• Board certification is mandatory.

• Active licenses in the relevant coverage areas.

• Capability to obtain and maintain multi-state licenses as needed.

• FNP or AGNP certification is preferred for nurse practitioners.

• Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home consultations, telehealth delivery, or similar formats is preferred.

• Availability for 40 hours per week.

• Ability to provide over 320 capacity units and complete more than 220 visits each month.

• Completion of ongoing mandatory training.

• Must have broadband connectivity with video and audio capability.

• A quiet environment with a neutral background for conducting visits is essential.

• Ability to maintain health plan member privacy in accordance with CVS Health guidelines.

• Strong verbal, written, and electronic communication abilities.

• Capability to work independently and practice autonomously.

• Proficiency in information management and technology.

• Ability to sit, write, use a computer and phone, communicate clearly, and hear member responses.

• Capacity to view and read computer screens, iPads, and other electronic devices.
