Virtual Clinician – Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Colorado.

📋 Description

• Conduct approximately 30-minute online Video Visits with members enrolled in Medicare Advantage, Medicaid, and select commercial plans.

• Review medications and medical histories during Video Visits.

• Educate members about their health and respond to their inquiries.

• Assist in closing gaps in care and connect individuals to the appropriate services.

• Provide 40 hours of availability each week and maintain a capacity of over 320 units per month.

• Ensure a strong visit completion rate with over 220 completed visits per month.

• Comply with training, affiliation, and Video Visit requirements.

• Obtain training and credentialing for health plan clients and lines of business in primary coverage states.

• Engage in regular meetings with managers, teams, and clinical staff.

• Meet operational, clinical, and member-experience KPIs and metrics set by Signify Health.

• Conduct Video Visits remotely while ensuring member privacy and adhering to required working conditions.

⛳️ Requirements

• Full-time availability of 40 hours per week.

• Board-certified nurse practitioner or physician assistant.

• Active state licenses in the relevant coverage areas.

• Ability to acquire and maintain multi-state licenses in the coverage area as needed.

• FNP or AGNP certification preferred for nurse practitioners.

• Experience with Signify Health's In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or similar formats is preferred.

• Training and credentialing for all health plan clients and lines of business in primary coverage states.

• Regular involvement in manager, team, and clinical meetings.

• Minimum requirements for remote visit delivery, including broadband connectivity with video and audio capabilities.

• A private and quiet environment with a neutral background.

• Ability to maintain health plan member privacy in accordance with CVS Health guidelines.

• Strong verbal, written, and electronic communication skills.

• Ability to work independently and practice autonomously.

• Proficiency in information management and technology.

• Capability to sit, write, operate a computer and phone, communicate clearly, and hear member responses.

• Ability to view and read a computer screen, iPad, and other electronic devices.
