This is a fully remote position, open to applicants in Florida.
• Manage after-hours phone calls and effectively address callers’ needs.
• Offer telephone guidance and education tailored to patient requirements.
• Respond to telephone inquiries regarding the services provided by Chapters Health System.
• Advocate for and clarify the advantages of Chapters Health System’s offerings.
• Collect essential information and documentation to maintain comprehensive, up-to-date, and accurate patient records.
• Input and monitor patient referral data within the EMR system.
• Provide reassurance to patients and families via phone communication.
• Address inquiries or identify situations that necessitate an in-person visit.
• Organize in-person visits as required or requested.
• Support patients and families in attaining desired comfort levels.
• Utilize support and expert resources to manage complex situations effectively.
• Record interactions with patients and families in the EMR and liaise with the Interdisciplinary Group.
• Notify leadership of technical issues and strive for resolution within 30 minutes.
• Report to a designated company location during emergencies when necessary.
• Undertake additional assigned tasks as needed.
• Must possess a current Florida RN license or a multi-state RN license.
• Capability to apply for or obtain a multi-state RN license within 30 days of employment if not already acquired.
• Ability to apply for or secure an RN license in Washington, D.C. within 30 days of employment.
• Minimum of one year of nursing experience is required.
• Preferred experience in hospice or hospital settings.
• Previous experience with an EMR/EHR system is necessary.
• Effective communication skills in English, both verbal and written.
• Bilingual in Spanish is preferred.
• Reliable transportation is essential to meet patients’ needs.
• Strong critical decision-making, problem-solving, clinical, and assessment abilities.
• Proficient in using equipment with visual and auditory components.
• Excellent time-management and prioritization skills.
• Capability to handle stressful situations in a professional and composed manner.
• Strong skills in patient service, information-seeking, listening, and knowledge transfer.
• Ability to thrive in a fast-paced environment and work collaboratively within a team.
• Proficient in computer operations, MS Office, online research, data entry, and system navigation.
• Availability to work scheduled shifts covering extended operating hours.
• Capacity to perform sedentary work with frequent interruptions.
• Ability to manage the emotional stress associated with caring for terminally ill patients and their families.
• Capable of performing essential physical tasks, including lifting patients/equipment, bending, pushing/pulling, and kneeling, with or without reasonable accommodation.
• Successful completion of competency requirements is mandatory.
• Consent to post-offer and ongoing drug/alcohol testing is required.
• Must maintain ongoing eligibility through the Florida Care Provider Background Screening Clearinghouse.
