Triage Nurse – RN

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver prompt and compassionate after-hours RN telephone triage services to patients and caregivers affiliated with hospice and home health partners.

• Demonstrate empathy and understanding during interactions, particularly when callers are facing challenging situations.

• Evaluate patient requirements and offer suitable care instructions.

• Collaborate with other healthcare team members to ensure coordinated care.

• Record patient care details in the electronic health record (EHR).

⛳️ Requirements

• Possession of an active RN license in a compact state is mandatory.

• A minimum of 4 years of clinical nursing experience is required. While experience in home health, palliative care, med-surg, geriatrics, ICU, ER, and/or hospice care is preferred, it is not essential.

• Fluency in English is required; Spanish language skills are an advantage.

• Proficient in using computers and telecommunications equipment.

• Capable of working independently as well as collaboratively within a team.

• Willingness to work flexible hours, including evenings and weekends.

• Must successfully pass a background check and drug test as part of pre-employment screening.

• At least 400 work hours in the past two years are required.
