This is a fully remote position, open to applicants in Pennsylvania.
• Travels within the designated territory to perform in-home assessments, which encompass thorough annual wellness exams, targeted screenings, and other visits in both patients' homes and virtual settings.
• Willingness to travel out of state for up to 100% of the time as required during the business week.
• Provides counseling and education to patients and their families regarding available benefits and programs aimed at promoting healthier lifestyles.
• Accurately documents essential information including chief complaints, relevant diagnoses, medical, family, and social history, review of systems, examinations, medications, allergies, assessments, and care plans.
• Responsible for coordinating care with primary care providers, specialists, and other necessary ancillary services.
• Ensures all documentation and paperwork are completed promptly.
• Upholds quality of care standards as defined by the practice.
• This role is remote within the state of employment, requiring attendance at periodic training and meetings outside of that state.
• Delivers evidence-based, timely care that minimizes avoidable hospitalizations, enhances quality of life, and prioritizes patient health and satisfaction.
• Prescribes medications, orders tests, and collaborates with the patient's Monogram physician.
• Performs effectively, as demonstrated by improved patient quality outcomes, which will be measured and reported daily.
• Helps close gaps in care by educating patients about preventive monitoring and collaborating with physician practices to schedule necessary diagnostic testing.
• Assists patients in accessing educational videos for better health management.
• Participates in integrated care team meetings.
• Possesses knowledge of disease diagnosis and prevention.
• Conducts assessments of patients' health statuses.
• Develops treatment plans tailored to patient needs.
• Implements plans that align with the appropriate care protocols.
• Follows up and evaluates patients' health statuses.
• Performs additional duties as assigned.
• Valid and unrestricted Registered Nurse and Nurse Practitioner Board certification for the relevant licensure (NP: ANCC/AANP).
• Current and unrestricted DEA certificate.
• Willingness to travel up to 100% of the time.
• Ability to work independently without direct supervision and practice autonomously.
• Access to reliable transportation, a valid driver’s license, and car insurance.
• Proficient with medical instruments and equipment necessary for the role.
• Familiarity with computer-based data management systems, medical records, and point-of-interview technology.
• Effective communication skills in both verbal and written forms, capable of engaging with retail and medical partners, patients, families, physicians, and community representatives.
• Comprehensive understanding of all federal and state regulations, including HIPAA and OSHA.
• Experience in Managed Care/IPA/Health Plans.
• Background in conducting wellness visits, focused assessments, or similar comprehensive visits, either virtually or in-home.
