Transitions of Care Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform thorough evaluations of patients moving from hospital care to home care.

• Create and execute tailored care plans in partnership with healthcare teams.

• Track patient advancements and modify care plans as needed to ensure the best possible outcomes.

• Provide education to patients and their families regarding post-discharge care and available resources.

• Foster communication among patients, caregivers, and healthcare professionals.

• Record all patient interactions and care plans in the electronic health record (EHR).

• Engage in quality improvement projects to enhance the delivery of care.

• Remain informed about best practices in transitions of care and healthcare regulations.

⛳️ Requirements

• Current RN license in the state of Georgia.

• At least 2 years of nursing experience, ideally in acute care or case management.

• Strong understanding of transitions of care processes and patient-centered care principles.

• Exceptional communication and interpersonal abilities for effective patient engagement.

• Proficient in electronic health record (EHR) systems.

• Capacity to work autonomously and manage time efficiently in a remote environment.

• Empathetic demeanor with a dedication to enhancing patient health outcomes.

• Certification in case management (CCM) or transitions of care is preferred.
