Tennessee Licensed Therapist – LCSW, LPC, LMFT

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Evaluate, devise, and execute care strategies tailored to each member, focusing on the most suitable and least restrictive care level.

• Collaborate with members, their families, and healthcare professionals to create a personalized care plan.

• Conduct individual counseling sessions and/or group therapy for both adolescents and adults.

• Engage in in-person or fieldwork as necessary for families involved with TN DCS.

• Identify and initiate referrals to social service programs including financial, psycho-social, community, and state support services.

• Advocate for members and their families as required, ensuring their needs and preferences are fully acknowledged and supported by the healthcare team.

• Apply approved clinical criteria to evaluate and ascertain the suitable level of care.

• Maintain comprehensive documentation of all member assessments, care plans, and referrals made.

• Meet established goals and Key Performance Indicators (KPIs).

• Familiarize oneself with the Substance Use Disorder Program model, taking responsibility for outcome-focused care.

• Participate in interdisciplinary team meetings and utilization management rounds, providing necessary information to facilitate a safe transition of care.

• Encourage responsible and ethical management of company resources.

• Uphold excellent punctuality and attendance during work hours.

• Execute other assigned duties as necessary.

⛳️ Requirements

• LCSW, LPC, or LMFT licensure in Tennessee.

• Preferred experience working with the substance abuse population.

• Background in maternal substance abuse is preferred.

• Experience working with both adults and adolescents.

• Familiarity with Medicare and Medicaid processes.

• Experience with remote work.
