This is a fully remote position, open to applicants in Tennessee.
β’ Evaluate, devise, and execute care strategies tailored to each member, focusing on the most suitable and least restrictive care level.
β’ Collaborate with members, their families, and healthcare professionals to create a personalized care plan.
β’ Conduct individual counseling sessions and/or group therapy for both adolescents and adults.
β’ Engage in in-person or fieldwork as necessary for families involved with TN DCS.
β’ Identify and initiate referrals to social service programs including financial, psycho-social, community, and state support services.
β’ Advocate for members and their families as required, ensuring their needs and preferences are fully acknowledged and supported by the healthcare team.
β’ Apply approved clinical criteria to evaluate and ascertain the suitable level of care.
β’ Maintain comprehensive documentation of all member assessments, care plans, and referrals made.
β’ Meet established goals and Key Performance Indicators (KPIs).
β’ Familiarize oneself with the Substance Use Disorder Program model, taking responsibility for outcome-focused care.
β’ Participate in interdisciplinary team meetings and utilization management rounds, providing necessary information to facilitate a safe transition of care.
β’ Encourage responsible and ethical management of company resources.
β’ Uphold excellent punctuality and attendance during work hours.
β’ Execute other assigned duties as necessary.
β’ LCSW, LPC, or LMFT licensure in Tennessee.
β’ Preferred experience working with the substance abuse population.
β’ Background in maternal substance abuse is preferred.
β’ Experience working with both adults and adolescents.
β’ Familiarity with Medicare and Medicaid processes.
β’ Experience with remote work.
