Telephonic Nurse Care Associate

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Perform telephonic clinical outreach to patients participating in Remote Patient Monitoring (RPM) and chronic care initiatives.

• Assess biometric alerts and identify readings that fall outside of the normal range; escalate clinical issues following established protocols.

• Deliver patient education and coaching to foster adherence to care plans and lifestyle recommendations.

• Ensure accurate documentation of patient interactions in EMR/RPM systems, adhering to clinical and regulatory standards.

• Collaborate with clinical management, RN Care Managers, healthcare providers, and interdisciplinary care teams to facilitate patient care.

• Assist in patient engagement efforts aimed at enhancing monitoring compliance and clinical outcomes.

⛳️ Requirements

• Must possess an active LPN/LVN license in good standing.

• Minimum of 5 years of clinical experience, including at least 3 years in care coordination, telehealth, home care, or chronic disease management.

• Preferred experience in supporting patients with conditions such as CHF, COPD, diabetes, or hypertension.

• Excellent telephonic communication, patient education, and motivational interviewing abilities.

• Proficient in using EMR/EHR systems and comfortable working in a technology-driven care environment.

• Capable of working independently within a structured remote clinical workflow.
