This is a fully remote position, open to applicants in Vermont.
• Deliver exceptional, continuous care for patients utilizing a telemedicine-first approach.
• Perform two-way video and audio consultations with patients.
• Collaborate effectively with patient care advocates and other healthcare team members to ensure patient care and follow-up.
• Participate in a fee-for-service model with comprehensive incentive programs for high-performing providers.
• Graduated from an accredited nursing or medical program with national certification.
• Currently licensed to practice in Vermont, with the capability to acquire additional state licenses as required.
• Maintain a valid Vermont DEA Registration in good standing, covering all schedules for prescribing medications, including Schedule II.
• A minimum of 2 years of post-residency practice experience is highly preferred.
• Must hold Board Certification in Family or Internal Medicine.
• Demonstrated commitment to evidence-based medicine.
• Eagerness to learn how to diagnose and manage a variety of adult and primary care conditions.
• A PC with Windows 11 Pro or a Mac model released after 2020 is necessary.
• Flexibility to create your own recurring schedules, including evenings and weekends.
• Backed by a strong team of patient care advocates and a licensed medical team.
