Telehealth Registered Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee and manage patients with heart failure from a remote setting.

• Monitor and analyze vital signs trends.

• Conduct virtual assessments of patients.

• Record patient interactions and interventions accurately.

• Collaborate with the clinical care team to ensure coordinated patient care.

• Implement triage protocols to evaluate patients and escalate care as needed.

• Perform virtual nursing assessments regarding symptoms, medication compliance, and disease progression.

• Provide education to patients and caregivers about heart failure medications, adherence practices, and potential side effects.

• Track vital signs and survey responses to identify any changes in condition and document those findings.

• Manage incoming calls by answering, directing, and following up as necessary.

• Identify urgent or emergent situations, seek assistance from medical providers, and initiate appropriate emergency interventions as instructed.

⛳️ Requirements

• Bachelor's Degree in Nursing is required.

• At least 3 years of experience in cardiology, heart failure management, or critical care nursing.

• Strong expertise in managing heart failure patients, including knowledge of GDMT (Guideline-Directed Medical Therapy).

• Current licensure as a Registered Professional Nurse in a compact state and in California; a California license is essential.

• Experience in patient phone triage, either in-office or through telemedicine.

• Capability to learn and effectively use various EMRs and communication applications, including Google products.

• Comfort with the virtual use of episodic vital sign monitoring devices.

• Ability to work from home in a HIPAA-compliant setting is necessary.

• Availability to work on certain holidays may be required.

• Advanced certifications (CCRN, CHFN, RN-BC) are highly preferred.

• Bilingual fluency in English and Spanish is highly preferred.
