This is a fully remote position, open to applicants in Florida, +6 more states.
• Conduct virtual evaluations of patients entering the program and assist in developing a care plan tailored to individual patient needs and established protocols.
• Prescribe and adjust medications in accordance with evidence-based guidelines and company-approved standards and regulations.
• Enhance guideline-recommended medical therapy for patients across all stages of heart failure.
• Address alerts escalated by the nursing team concerning deviations in patients’ vital signs, physiological data, and symptoms.
• Manage changes in clinical status independently that fall within the APP’s scope of practice while referring out-of-scope clinical changes to a physician.
• Collaborate with the nursing team to ensure that appropriate medications, lab tests, and virtual appointments are ordered and communicated effectively to the patient.
• Educate patients on healthy behaviors and self-management practices.
• Providers will be accountable for fostering meaningful and measurable improvements in disease control rates, including reductions in hospitalization.
• A minimum of 3 years as a Board-Certified Adult or Family Nurse Practitioner, with prior experience in adult chronic care management, particularly in cardiology and heart failure.
• Completion of an accredited NP program accompanied by a national certification.
• Active licensure in two or more of the following states is mandatory: FL, TN, TX, NC, OH, KY, and VA.
• Willingness to obtain additional state licenses as necessary.
• Required prior experience in the management of heart failure patients, including the prescription and management of evidence-based therapies.
• Previous telehealth experience is highly preferred.
• A proactive attitude and self-motivated work ethic that aligns with the core values and objectives of the company.
• Desire to thrive in a fast-paced, rapidly changing environment with a strong adaptability to change.
• Capability to work from home in a HIPAA-compliant environment is essential.
• Proficiency in learning and utilizing various EMRs and communication applications, including Google products.
