Telehealth Nurse Practitioner – Heart Failure

This is a fully remote position, open to applicants in California.

📋 Description

• Conduct virtual evaluations of patients entering the program and assist in formulating care plans tailored to individual patient needs and established protocols.

• Address alerts escalated from the nursing team concerning deviations in vital signs, physiological data, symptoms, and changes in weight.

• Independently handle clinical-status changes within the scope of practice of the Advanced Practice Provider (APP).

• Refer any clinical changes that fall outside the scope of practice to the lead Medical Doctor (MD).

• Collaborate with the nursing team to ensure the ordering and communication of medications, labs, and virtual appointments.

• Work alongside the nursing team to reconcile medications at both program entry and exit.

• Act as the primary contact for patients’ primary care teams, providing customized information.

• Educate patients on healthy lifestyle choices and self-management strategies.

• Manage an expanding patient panel and assist in the development of a telemedicine program utilizing medication-management titration algorithms and patient-management strategies.

⛳️ Requirements

• Minimum of 2 years as a Board-Certified Nurse Practitioner with a focus on heart failure and/or internal medicine in either an ambulatory or inpatient setting.

• Active nursing license in California.

• Willingness to obtain additional state licenses as necessary.

• Proficiency in both English and Spanish is highly desirable.

• Previous experience managing complex chronic care programs is strongly preferred.

• Prior telehealth experience is strongly preferred.

• Openness to working in a dynamic, evolving environment within a growing organization.

• Ability to work from home in compliance with HIPAA regulations is essential.

• A strong desire to provide health education and skills in health promotion techniques.

• Capability to learn and effectively use multiple Electronic Medical Records (EMRs) and various communication applications, including Google products.

• Availability for full-time work from Monday to Friday, 8:00 AM to 4:30 PM PST.
