Telehealth Nurse, LPN/LVN

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct regular telehealth consultations for a designated group of members.

• Provide accurate and confident medical, dietary, and wellness guidance to members.

• Ensure documentation is both accurate and timely.

• Build and maintain a positive rapport with members and nursing staff.

• Capable of delivering high-quality, high-volume calls.

• Drive clinical results through member engagement and coordination.

• Refer members to our Provider team when necessary.

• Offer device support and troubleshooting in a knowledgeable and professional manner.

• Provide support and encouragement as required.

• Maintain strong relationships with members, the nursing team, and providers.

• As a team member, promote and nurture a culture of kindness, compassion, and respect.

• Assist with special projects and additional assignments as needed.

⛳️ Requirements

• Must possess a current, active, unrestricted LPN/RN multi-state compact license in a nursing compact state and maintain good standing with the Board of Registered Nurses.

• Requires a minimum of 2 years of hands-on nursing experience; experience in telehealth and chronic care management is highly preferred.

• EHR experience, particularly with AthenaHealth, is advantageous.

• Fluency in both English and Spanish is strongly preferred.

• Comfortable using a computer; experience navigating multiple applications during calls is essential; Macbook proficiency is a plus.

• Must have dependable high-speed internet access.

• Strong time management skills and a self-motivated attitude are required.

• Must be a quick learner in a dynamic, fast-paced environment.

• Professional and effective communication style that resonates with all team members and clients.

• Self-driven and capable of working independently.
