Telehealth Heart Failure Registered Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Assess patients utilizing established protocols to decide on nursing interventions and determine when to escalate care to the appropriate level.

• Conduct virtual nursing evaluations to assess patient symptoms, adherence to medications, and the progression of diseases.

• Educate patients and caregivers about heart failure medications, adherence strategies, and possible side effects.

• Monitor patient vital signs and survey feedback, identify any changes in condition, and accurately document findings.

• Record all interactions and nursing interventions thoroughly.

• Manage, redirect, and follow up on incoming calls effectively.

• Identify urgent or emergency situations, obtain assistance from medical providers, and initiate appropriate emergency interventions as instructed.

⛳️ Requirements

• A Bachelor’s Degree in Nursing is required.

• A current, active, and unrestricted license to practice as a Registered Professional Nurse in a compact state, in good standing with the Board of Registered Nurses.

• A minimum of 3 years of experience in cardiology, heart failure management, or critical care nursing is essential.

• Strong expertise in heart failure patient management, including familiarity with Guideline-Directed Medical Therapy (GDMT).

• Advanced certification is highly preferred (such as CCRN, CHFN, RN-BC).

• Experience with patient phone triage, whether in-office or through telemedicine, is required.

• Capacity to learn and effectively utilize various EMRs and communication applications, including Google products.

• Comfort with the virtual use of episodic vital sign monitoring devices is necessary.

• Must be able to work from home in a HIPAA-compliant setting.

• Proficiency in both English and Spanish is highly preferred.

• Availability to work on some holidays may be required.
