This is a fully remote position, open to applicants in United States.
• Oversee care coordination efforts for designated members to guarantee appropriate services and ensure continuity of care.
• Manage the daily functions of care coordination team members and resolve member/provider concerns.
• Review and monitor care coordination documentation, transition-of-care records, and service evaluations.
• Allocate caseloads in accordance with state regulations, contractual obligations, and the needs of members.
• Work collaboratively with the care management team to provide effective and suitable care for members.
• Ensure compliance with contractual and regulatory standards through established practices and policies.
• Provide education, guidance, and resources to the care coordination team.
• Assess team performance and give constructive feedback regarding performance, objectives, and career achievements.
• Mentor the team to enhance continuity of care across various services.
• Support the onboarding, recruitment, and training of care coordination team members.
• Manage telephonic, digital, home, and other outreach initiatives to evaluate member needs and collaborate with available resources.
• Ensure compliance with service-level agreements, quality standards, and KPI expectations.
• Take a leadership role in driving change within the defined scope of responsibility.
• Travel for in-field training, regional team gatherings, and leadership meetings.
• Bachelor's degree and a minimum of 4 years of relevant experience.
• Equivalent experience gained through applicable knowledge, responsibilities, and skills may be considered.
• For Iowa candidates: Bachelor's degree with 4+ years of experience with served populations, or RN with 6+ years of experience with served populations.
• For the Arkansas Total Care plan: a valid driver's license is required.
• A child and adult maltreatment check is mandatory prior to hiring and on a recurring basis.
• A drug screening will be required at the time of hire and periodically thereafter.
• Candidates must reside in Arkansas or a neighboring city.
• 20% travel is required.
• This is a remote position within a specific geographic area, which includes local community travel.
• Experience with Behavioral Health and/or Developmental Disabilities populations is highly preferred.
• Availability is required from Monday to Friday, between 8:00 AM and 5:00 PM.
