This is a fully remote position, open to applicants in Nevada.
• Perform thorough assessments of patients to determine their clinical, educational, and psychosocial requirements.
• Deliver patient education in both Spanish and English regarding the management of chronic diseases, medications, nutrition, preventive care, lifestyle changes, and self-management techniques.
• Create and update personalized care plans in collaboration with the interdisciplinary care team.
• Coordinate care among providers, specialists, pharmacies, and community resources to ensure seamless continuity of care.
• Oversee Remote Patient Monitoring (RPM) data, recognize trends or clinical issues, and escalate findings as necessary.
• Enroll qualifying patients in CCM and RPM programs while ensuring adherence to CMS program guidelines.
• Accurately record all patient interactions and care coordination activities in the electronic health record (EHR).
• Establish trusting relationships with patients through proactive outreach and continued engagement.
• Uphold patient confidentiality and adhere to HIPAA and organizational policies.
• Active, unrestricted Nevada Licensed Practical Nurse (LPN/LVN) license in good standing or the willingness to obtain a Nevada license.
• At least 2 years of clinical nursing experience.
• Proficiency in both Spanish and English.
• Strong understanding of chronic disease management and principles of patient education.
• Experience with Electronic Health Records (EHR) and healthcare technology platforms.
• Exceptional communication, organizational, and time-management abilities.
• Capacity to work independently in a remote setting while managing multiple patient interactions.
• A dedicated, HIPAA-compliant home office equipped with reliable high-speed internet.
• Preferred: Prior experience with Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Transitional Care Management (TCM), or value-based care programs.
• Preferred: Experience with Medicare beneficiaries.
• Preferred: Familiarity with CMS documentation requirements.
• Preferred: Background in telehealth or virtual patient care.
