RN Case Manager, Experienced

This is a fully remote position, open to applicants in Maine, +4 more states.

📋 Description

• Oversee and facilitate high-quality, cost-efficient care for patients with ESRD and CKD.

• Reduce fragmentation in the healthcare delivery system.

• Coordinate care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager.

• Operate autonomously while collaborating with the healthcare team.

• Manage clinical, financial, and patient satisfaction outcomes.

• Act as a change agent, adapting practice patterns to new electronic processes and productivity standards.

• Mentor healthcare performance.

• Utilize Capella and other DaVita-specific clinical software programs to enhance care management.

• Work in a telephonic, remote-based role from a home office.

⛳️ Requirements

• A.D.N. degree from an accredited nursing school is required.

• B.S.N. is preferred.

• A three-year diploma from an accredited diploma program may be accepted in lieu of a nursing degree.

• A current Registered Nurse (RN) license in the state of practice is mandatory.

• Continuing education credits must be maintained as required by the state of practice.

• At least five (5) years of clinical nursing experience is required.

• A minimum of three (3) years of renal nursing experience is preferred.

• Proven knowledge and understanding of data management related to clinical, financial, and patient satisfaction outcomes.

• Demonstrated effectiveness in a change agent role.

• Strong knowledge and understanding of Continuous Quality Improvement (CQI) techniques.

• Previous experience in healthcare performance coaching is required.

• Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) is preferred.

• Current CPR certification is required.

• Ability to adjust personal practice patterns to accommodate new/electronic processes and heightened productivity expectations related to Capella implementation.

• Must possess a valid driver’s license in the state of employment, with a positive driving record and the ability to meet insurance coverage requirements.

• Basic computer skills and proficiency in MS Word and Outlook are required.

• Functional proficiency with DaVita-specific clinical software programs, including Capella, must be achieved within 90 days of employment.

• A home office with internet connectivity that meets a minimum of 1MB upload and 1MB download speed is required.
