RN, Care Manager

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Provide targeted, organized, coordinated, cost-effective, and patient-focused care.

• Collaborate with patients, families, caregivers, and healthcare providers.

• Work alongside interdisciplinary staff and stakeholders throughout the care continuum.

• Enhance patients’ overall health through strategies focused on disease prevention and management.

• Implement evidence-based interventions tailored to patient populations.

• Facilitate disease-specific quality measures, enhance patient satisfaction, improve access to services, and optimize hospital/ED utilization outcomes.

⛳️ Requirements

• Bachelor’s Degree.

• APRN - Advanced Practice Registered Nurse License issued by the State of Ohio Board of Nursing.

• BLS - Basic Life Support certification from the American Heart Association.

• RN - Registered Nurse license from the Ohio Board of Nursing.

• Capability to work 40 scheduled hours per week during the day shift.
