This is a fully remote position, open to applicants in United States.
• Oversee patient transfer logistics in accordance with client-defined workflows to facilitate patient acceptance and bed placement.
• Determine the suitable level of care, accepting provider, consultation, and access for referrals from hospitals, clinics, extended care facilities, and physician offices.
• Collaborate directly with clinicians to convey accurate clinical presentations, ensure timely access and bed assignments, and document precise patient information.
• Employ critical thinking and clinical decision-making skills to create appropriate destination care plans.
• Utilize EHR, telephony, and office systems to enhance operations, ensuring accurate data entry and safeguarding PHI.
• Provide recommendations based on clinical data, patient or physician preferences, as well as the capabilities and capacities of transferring and receiving facilities.
• Serve as a liaison among physicians, hospital staff, external agencies, patients, and client facilities.
• Monitor compliance and performance for both individuals and teams.
• Support changes in processes and initiatives related to regulations and quality assurance.
• Apply knowledge of EMTALA, Ethics and Compliance, quality initiatives, and HIPAA regulations.
• Report safety, compliance, and operational risks through the established chain of command, suggesting resolutions or improvements.
• Collaborate with providers and healthcare professionals to ensure timely acceptance, access to appropriate care, and bed assignments.
• Engage in process improvement, professional development, peer review, and shared leadership.
• Assist with the collection of quality data, identify trends, and report barriers to patient access and throughput.
• Perform additional tasks and analysis as requested or directed.
• Current Registered Nurse (RN) licensure — required.
• Active multistate/compact RN licensure, if applicable based on state of residence — required.
• Associate Degree in Nursing or diploma (ADN) — required.
• Minimum of 1 year of acute care experience — required.
• Must maintain active RN licensure in the states where transfer center services are offered.
• Designated workspace with a locked door, in compliance with HIPAA regulations.
• Recommended secondary work location that adheres to HIPAA regulations for power outages.
• Minimum internet speed of 100 Mbps download and 20 Mbps upload at both primary and secondary work locations.
• Proficient in EHR data entry, telephony systems, and office systems.
• Familiarity with Microsoft Office tools, including Outlook, MS Teams, Excel, and Word.
• Capability to review and interpret patient transfer case information.
• Ability to escalate or inform others regarding changes in patient status.
• Competence in documenting tasks and conversations related to patient transfers.
• Strong critical thinking and clinical decision-making abilities.
• Exceptional oral and written communication skills.
• Bachelor’s degree in nursing (BSN) — preferred.
• Over 3 years of acute care experience — preferred.
• Experience in a healthcare contact center — preferred.
• Familiarity with Epic electronic health record — preferred.
• Experience with Genesys contact center — preferred.
• Ability to work assigned shifts, including days, evenings, nights, every other weekend, rotating holidays, and on-call coverage.
