Registered Nurse – Telehealth/SUD

📋 Description

• This role is fully remote; however, candidates must reside in one of the following states: AL, AK, AZ, CO, FL, GA, ID, IN, KS, MD, MI, NH, NM, NY, NC, OH, OR, PA, TN, TX, WA, WY.

• The Registered Nurse collaborates directly with patients to provide medication-assisted recovery for substance use disorder (SUD), supporting them in their recovery journeys alongside the Care Team, which includes Nurse Practitioners, Medical Assistants, and Peer Recovery Specialists.

• Our team is committed to delivering compassionate and empathetic engagement and monitoring through telephonic, text-based, and video-based (telehealth) methods.

• Engage with patients compassionately via telephone, text, and video (Telehealth), using clinical decision support tools to enhance monitoring and interaction.

• Assist patients within the Boulder application to ensure successful engagement and the initiation of treatment.

• Educate patients about addiction, medications, and available support services, addressing questions regarding treatment and program protocols while providing guidance to both patients and providers within the scope of practice.

• Deliver education on opioid overdose prevention, education, and reversal, as well as safer consumption and safer sex practices, including testing.

• Access information from the Prescription Drug Monitoring Program and facilitate monitoring protocols for patient adherence for each assigned individual.

• Evaluate progress, patient functioning, and stages of recovery; collaboratively develop and update individualized treatment care plans with care team members and the patient.

• Assess and advocate for management of acute withdrawal symptoms.

• Evaluate pain and share findings with the Clinician.

• Provide education on Buprenorphine initiation and follow up with patients accordingly.

• Ensure effective, proactive, and superior communication with patients, providers, and the care team regarding follow-up.

• Maintain accurate and current medical records of individual patient progress during all clinical interactions.

• Identify and provide crisis intervention within the scope of practice.

• If applicable, facilitate placements for adjunctive or higher levels of care and maintain communication with these placements to ensure continuity and smooth transitions of care.

• Obtain and review drug testing results, recommending further testing as necessary, guided by clinical decision support tools.


⛳️ Requirements

• Bachelor’s Degree in Nursing is preferred.

• Preference will be given to candidates with 2+ years of specialized nursing experience in outpatient substance use disorder treatment.

• A harm reduction philosophical approach to care is essential.

• Candidates must demonstrate compassion, empathy, and a strong dedication to patient care.

• Excellent written and verbal communication skills are required.

• Outstanding customer service abilities, computer proficiency, and record-keeping skills are important.

• Ability to manage multiple tasks simultaneously and prioritize based on the severity of need while working independently with minimal supervision is necessary.

• Capability to achieve specified outcomes in collaboration with all leadership team members is vital.

• Demonstrated quality in clinical performance, evidenced by creative problem-solving and interdisciplinary communication, is expected.

• Experience in working collaboratively with medical staff, including Clinicians, Care Advocates, Peers, Case Managers, and colleagues across all aspects of care provision is preferred.

• Experience in office-based nursing is a plus.

• Knowledge of Hepatitis C and HIV is preferred.

• Membership in a relevant professional nursing organization (e.g., International Nurses Society on Addiction) or specialty certification (e.g., CARN) is preferred.

• A dedicated, private workspace with a lockable door and high-speed internet is required to maintain a secure, distraction-free environment, ensuring compliance with HIPAA and confidentiality standards.

• Due to privacy policies, we cannot employ individuals who are or have previously been patients at Boulder Care.
