Registered Nurse, RN Case Manager

This is a fully remote position, open to applicants in Washington.

📋 Description

• This role operates independently while also collaborating with all healthcare team members to ensure the coordination and delivery of quality, cost-effective care, minimizing fragmentation in the healthcare delivery system for patients with ESRD and CKD.

• The position entails coordinating care among the patient, family, physician, provider, care teams, community, and Regional Operations Manager.

⛳️ Requirements

• An A.D.N degree from an accredited nursing school is required; a B.S.N is preferred, and a three-year diploma from an accredited diploma program may be accepted in lieu of a nursing degree.

• A current Registered Nurse (RN) license in the state of practice is mandatory.

• Maintenance of continuing education credits as required by the state of practice is necessary.

• A minimum of five (5) years of clinical nursing experience is required.

• At least three (3) years of renal nursing experience is preferred.

• Strong preference for case management experience, with proven ability to assess patient needs, develop and manage individualized care plans, coordinate services across interdisciplinary teams, and assist patients through complex healthcare transitions.

• Certification as a Nephrology Nurse (CNN) or Certified Case Manager (CCM) is preferred.

• Proven knowledge and understanding of data management related to clinical, financial, and patient satisfaction outcomes.

• Demonstrated effectiveness and experience in a change agent role.

• Knowledge and understanding of Continuous Quality Improvement (CQI) techniques are required.

• Prior experience in healthcare performance coaching is required.

• A current CPR certification is essential.

• Ability to adapt personal practice patterns to new/electronic processes and increased productivity expectations related to Capella implementation.

• A current driver's license in the state of employment with a positive driving record and the ability to meet insurance coverage requirements is required.

• Basic computer skills and proficiency in MS Word and Outlook are required.

• Functional proficiency with DaVita-specific clinical software programs, including Capella, is required within 90 days of employment.

• A home office with internet connectivity of at least 1MB upload and 1MB download speed is required.
