This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Engage with members who qualify for the TCL special needs population to provide information on permanent supportive housing, available supports, services, and other community resources.

• Collaborate with Diversion staff, Transition Coordinators, and the Next Step Rehousing Team to assist in transition planning and advancement through the TCL process for members seeking permanent supportive housing in the community.

• Work alongside the Trillium Integrated Care Nurse, OT, COTA, and Clinician to assess complex care requirements and link clinical services.

• Promote community integration, connect members to services, and offer education and support for those who opt not to transition to permanent supportive housing in the community.

• Employ person-centered planning techniques to collect information and build relationships with members and their legally responsible persons to effectively address individual needs.

• Educate and support members and their guardians in understanding and exercising their rights and responsibilities.

⛳️ Requirements

• High School Diploma or GED.

• Must have at least one year of lived experience with a behavioral health condition and/or substance use.

• Lived Experience is characterized as having lived in recovery from mental illness and/or substance use disorder.

• Certification as a North Carolina Peer Support Specialist.

• A valid driver’s license is required.

• Must reside within Trillium’s Southern Region, which encompasses the following counties: Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland.

• Ability to travel within the catchment area as needed.
