This is a fully remote position, open to applicants in Louisiana.
• Deliver systematic psychiatric consultations to Behavioral Health Care Managers (BHCMs) through Structured Case Review (SCR), focusing on: High-risk patients, Patients entering treatment, Individuals not achieving clinical targets, Complex diagnostic cases.
• Provide guidance to Primary Care Providers (PCPs) on: Evidence-based pharmacologic treatment recommendations, Strategies for diagnostic clarification, Lab monitoring and medication management considerations.
• Record treatment recommendations directly within partner electronic health record systems to ensure seamless care coordination.
• Be available for occasional urgent consultations with PCPs regarding complex patient cases, aiming for response times within one business day (or one hour for urgent needs).
• Leverage Cabana’s clinical registry and measurement-based care systems to: Monitor patient progress across assigned caseloads, Identify patients needing treatment escalation or care adjustments, Support population-level behavioral health management initiatives.
• Engage in weekly caseload review meetings with BHCMs and interdisciplinary care teams.
• Review validated outcome data using tools such as: PHQ-9, GAD-7, Columbia Suicide Severity Rating Scale (C-SSRS).
• Assist in treatment-to-target care planning and strategies for symptom remission.
• Act as a clinical mentor and psychiatric resource for PCPs and BHCMs.
• Provide advice on: Behavioral health treatment planning, Diagnostic conceptualization, Medication management strategies, Handling complex psychiatric cases.
• Collaborate closely with interdisciplinary care teams to enhance the integration of behavioral health services within primary care settings.
• Maintain effective communication and collaborative relationships across remote care teams.
• Partner with Cabana’s Clinical Director to: Create and refine psychiatric consultation workflows, Develop escalation pathways and consultation protocols, Set documentation standards for CoCM implementation.
• Aid in the development of: Standard operating procedures (SOPs), Clinical workflow frameworks, Quality assurance processes, Pilot program launch strategies.
• Take part in quality review meetings and provide psychiatric insights for operational and performance improvement discussions.
• Perform all other assigned duties.
• MD or DO with an active, unrestricted medical license in Louisiana.
• Board Certified in Psychiatry (ABPN or equivalent).
• At least 3+ years of clinical experience, preferably in one or more of the following areas: Integrated behavioral health, Primary care behavioral health, Consultation-liaison psychiatry, Collaborative Care (CoCM) environments.
• Strong understanding of evidence-based psychiatric medication management for common behavioral health issues, including: Anxiety disorders, Depression, PTSD, Behavioral health conditions with medical comorbidities.
• Experience in using measurement-based care tools, including: PHQ-9, GAD-7, C-SSRS.
• Familiarity with electronic health record (EHR) systems, such as Epic, Cerner, or other equivalent EHR platforms.
• Excellent written communication and clinical documentation abilities.
• Capacity to excel in a dynamic, fast-paced startup environment with shifting workflows and priorities.
• Must reside within the United States while maintaining active Louisiana medical licensure.
• **Strongly Preferred**
• Prior experience in the Collaborative Care Model (CoCM).
• Knowledge of “treat-to-target” protocols and registry-based population health management.
• Experience consulting in: Primary care settings, Federally Qualified Health Centers (FQHCs), Integrated care environments.
• Understanding of CoCM and Behavioral Health Integration billing requirements, including CPT codes: 99492, 99493, 99484.
• Comfort in managing population-based caseloads as opposed to traditional appointment-only psychiatric models.
• Strong collaboration and interdisciplinary communication skills.
• Proven ability to establish workflows and contribute to clinical program development initiatives.
