This is a fully remote position, open to applicants in Washington.
• Cultivate and strengthen relationships with senior Care Management leadership through continuous collaboration on initiatives aimed at improving operational efficiency, workflow management, and analytical support.
• Manage departmental projects by identifying key initiatives, creating project plans and timelines, and overseeing project execution until completion.
• Facilitate interdepartmental meetings, acting as a subject matter expert and educator to enhance cross-functional understanding of Care Management workflows and processes.
• When necessary, supervise and conduct regular internal audits of clinical staff performance, assessing compliance with Care Management standards, regulatory requirements, and organizational policies, while providing constructive feedback to staff leadership regarding performance gaps.
• Ensure that Care Management training adheres to all standards established by the National Committee for Quality Assurance, Healthcare Authority, Centers for Medicare and Medicaid Services, and other relevant regulatory bodies.
• Collaborate with leadership to design and implement new programs, as well as improve existing programs, including participation in and leadership of division-level initiatives alongside Care Management-specific activities.
• Engage in internal cross-functional teams to guarantee that care management programs comply with regulatory and compliance standards.
• Act as a resource and support for junior Program Managers, delegating tasks as appropriate to enhance team skill sets and balance workloads within the department.
• Serve as a liaison between data analytics personnel and Care Management leadership to create or modify care management reports that evaluate productivity and program outcomes.
• Generate care management reports and present findings to relevant leadership and committees.
• Identify opportunities to standardize and enhance care management processes, collaborating with leadership and staff to implement effective desk procedures across various business lines.
• Carry out other duties as assigned.
• Possess a bachelor’s degree or an equivalent combination of education and highly relevant experience.
• Have a minimum of five (5) years of experience in program management administration, which includes developing and/or implementing programs or leading teams that establish and execute programs within a healthcare environment.
• Hold at least three (3) years of experience in care management operations in a program management and/or process improvement role, along with one (1) or more years of mentoring, training, or teaching experience.
• Maintain current, unrestricted licensure in the State of Washington as a Registered Nurse or as a Licensed Independent Clinical Social Worker.
• Demonstrate proficiency in using Microsoft Office products, care management workflows, software applications, and reporting tools.
