Principal Care Management Nurse, LATAM

This is a fully remote position, open to applicants in Latin America.

📋 Description

• Maintain continuous communication with patients participating in the PCM program.

• Monitor patient conditions, which includes: Symptoms, Medication adherence, Progress, and response to treatment.

• Provide education to patients regarding their health condition and care plan.

• Assist in the execution and follow-up of care plans.

• Coordinate services to ensure seamless continuity of care.

• Identify any changes in patient status and raise concerns to the provider as necessary.

• Ensure patients adhere to treatment recommendations and attend scheduled appointments.

• Accurately record all patient interactions and updates within the system.

⛳️ Requirements

• Licensed Nurse or MD (mandatory).

• Proficient in English (C1 level or above) – exceptional spoken and written communication skills.

• Proven experience in working with medical offices based in the U.S.

• Familiarity with chronic care management, patient follow-up, or care coordination.

• Strong clinical judgment with the ability to assess patient conditions remotely.

• Experience in providing patient education and support.

• Highly organized with excellent documentation skills.

• Ability to work independently in a remote setting.

• Reliable computer (minimum 8GB RAM).

• Stable internet connection (minimum 100 Mbps).

• Quiet and professional working environment.
